Application for "Certification of Need for Nursing Care"

In order to use nursing care insurance services, you will need to be certified as needing nursing care by your city or town.
Please apply for nursing care certification at your local city hall. The application process can be completed by the person in question or by a family member.

Certification of nursing care level required

A visiting investigator from the city or town will conduct an interview survey about the person's physical and mental condition and determine the level of care required*.
*The level of care required refers to the degree of need for care. It is determined on a 1,2-level scale: "Support Required 1, 5" and "Care Level 7 to XNUMX."

Application for residence, nursing care consultation

Please contact us by phone to let us know that you would like to move into our facility. After discussing the date and time, please come to our facility to complete the application process. At that time, please let us know about your current physical and mental condition and any concerns you may have about nursing care.

Contract and move-in

After a preliminary survey and signing a contract, you will be able to move in. If there are many applicants, we will assign a score based on the information you provide. Please feel free to contact us with any questions or concerns you may have about using the facility.

A care manager or other relevant person will be asked to create a nursing care service plan (care plan).
